Power Perfomance Waiver

Please click on the button below in order to download our waiver for your child. If you have more than one child attending either session, a separate waiver is required. Once completed, please bring the completed waiver(s) with you to your child's first session. If we do not receive your child's waiver on his/her first day of training, they will not be permitted to participate in that day's session and no refunds will be granted for not turning in waiver(s), as required.
